Dear Service Providers and Volunteers:

I am writing to request your help with an important project.  As you may know, the city of Columbus has adopted a 10-Year Plan to End Homelessness and Home for Good: The Alliance to End Homelessness is charged with implementing the plan.  Our goal is to make homelessness atypical, temporary and non-recurring through strategic, long-term solutions.

Since it has been approximately two years since the plan was created, it is important to assess and understand the current needs of our community including the challenges and barriers to creating long-term solutions to end homelessness.  In order for us to effectively determine the needs and challenges, we would like to ask for your assistance in completing an online survey.

You were selected to be a part of this project because of the work you are doing to end homelessness.  Your candid and thoughtful reply will help go a long way in helping us establish the order of priority to guide our efforts in addressing this complex issue.

To complete the survey online, please go to:

The survey should take approximately 10 to 15 minutes to complete.  We ask that you complete it by Friday, July 13th.  Your response and any comments will be treated with utmost confidentiality.  After the results are tabulated and compiled, we will issue a report.

Thank you in advance for your participation in this important project. If you have any questions about the administration of the survey, or if you would like more information about our work, please contact me at or (706) 327-3255, extension 201.
